Personnel Changes (No Impact to Level or Salary)

A PAF must be prepared if there is a change to the following (with no impact to salary and level):

  1. Title change (must include a copy of the letter to employee).  Please note that title changes must be reviewed and approved by HRS first.
  2. Department account number
  3. Status, such as LWOP, administrative leave, sabbatical leave, etc.

If it is a change in supervisor, a copy of the notification letter (informing the employee of the change) or department memo should be sent to HRS.
